New patients

registering as a patient

Please bring in the completed and signed medical cards for each person requesting to join the practice. Your records will be applied for through the Health Authority.

You will be invited to attend for a new patient medical and will not be fully registered with the practice until you have had this medical.

The practice will refuse an application if you reside outside the practice area or if the list is closed. You will not be refused for any other reason. You will be asked for proof of identity.
