how to request a repeat prescription

Please give 48 hours notice.

You may re-order by:

• Ticking items needed on request slip (attached to your prescription) and bringing to the surgery.
   
• Post – send request slip (attached to your prescription) with SAE
   
• Fax – ticking items needed on request slip (attached to your prescription) and fax to the surgery on 0161 703 9111
   
• Online via Emis Access - Click here

Children under 14 will not be allowed to collect prescriptions.
